Dr Tracie O’Keefe DCH, BHSc, ND

To confirm your booking, please read the terms and conditions below, enter your contact details and submit your agreement.

TERMS OF PAYMENT

A credit card number is required at the time of booking. For the initial booking, you are required to pay by credit card for the very first two-hour session to secure the appointment. Payment will be deducted 7 DAYS before the time of your appointment from your credit card.

CANCELLATION POLICY
If you need to change or cancel your appointment you will need to make those alterations at least 7 DAYS in advance BY TELEPHONE ONLY: 0403 398 808.

Changes or cancellations made less 7 days in advance will incur the charge of the FULL rate for that session.

Missed appointments or late cancellations will be charged to your credit card, or your credit will be used up if you are in advance.

After you have completed this form, you will receive a Client Guide on How to Do Well in Therapy. You need to read this before attending your first appointment.
