To confirm your booking, please read the terms and conditions below, enter your contact details and submit your agreement.
TERMS OF PAYMENT
A credit card number is required at the time of booking. For the initial booking, you are required to pay by credit card for the very first two-hour session to secure the appointment. Payment will be deducted 48 business hours before the time of your appointment from your credit card.
If you need to change or cancel your appointment you will need to make those alterations at least 48 BUSINESS hours in advance (Mon-Fri 9am-5.00pm Australia Sydney time) BY TELEPHONE ONLY: 0403 398 808.
So if your appointment is on a Monday, you will need to cancel it on the Thursday before during business hours. If it is on a Tuesday, you will need to cancel the Friday before during business hours and so on.
Changes or cancellations made less than 48 business hours in advance will incur the charge of the FULL rate for that session. Missed appointments or late cancellations will be charged to your credit card, or your credit will be used up if you are in advance.
After you have completed this form, you will receive a Client Guide on How to Do Well in Therapy. You need to read this before attending your first appointment.
